    1. Like almost all aircraft mishaps, the pilot error was only one link in the mishap chain.

    2. Apparently this is the only USAF flying unit in the world without a STAN EVAL.

    3. Of all the aircraft EPs, loss of thrust on TO would have been the one for everyone to have memorized.

    Hindsight is close to 20/20 with a black box and survivor statements, right?

    No aircrew takes-off with a collective death wish. It was a very sad accident, resulting from an unbroken chain of errors.
    "Pilot error" was simply the last link in that chain.

    This should serve as a reminder to all onlookers that military aviation is full of risk, every single mission. Especially the innocuous "milk runs". Mother Nature and physics don't care.

    Very sad for their families and squadrons.

    The overall mentality of “its going to the boneyard” didn’t help. The maintenance guys who didn’t follow the T.O. procedure and not use the appropriate calibrated instrument for RPM’s on the engine that was low power...imho is unforgivable. Working on C-130’s for the past 14 yrs on Hurlbert, we civi’s get raked over the coals for deviating one bit.
